Our Policies

You, the consumer, hereby consent, authorize, and assign any and all rights to Synergy Hockey Management to photograph me, take motion pictures of me, take video footage of me, and/or make electronic sound recordings of me (herein referred to as photographic or electronic reproductions).

You authorize the use of any such photographic or electronic reproductions of me for any purpose, including, but not limited to educational and other public media as may be deemed appropriate by the Synergy Hockey Management (I understand that I may be identifiable from such photographic or electronic reproduction).

You further consent and authorize the above and others to release or circulate the same in any manner for any and all purposes in any form with or without my name or the name of the child.

You understand the photos, videos, film and/or audio recordings may be viewed by the general public and that other uses may be made of them. I further agree and consent that Synergy Hockey Management and others are not responsible for any misappropriation of the photos, videos, film and /or audio recordings by any member of the general public or anyone else.

In consideration of this participant being permitted to perform or engage in Synergy Hockey Management, and for other good and valuable consideration, the receipt and sufficiency of which are hereby acknowledged, the undersigned (participant’s legal guardian) on behalf on himself/herself or participant:

1. HEREBY REPRESENTS that, the participant who is under 18 years of age, the undersigned is the parent or legal guardian of the participant and has legal capacity and authority to enter into this agreement on behalf of the participant. I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I, ON BEHALF OF MYSELF OR PARTICIPANT, HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND HAVE SIGNED IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENTS, ASSURANCES, OR GUARANTEES BEING MADE AND INTEND MY SIGNATURE TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT PERMITTED BY LAW.

2. I have read and understand the payment process, communication, and refund policy of the contract and authorize Synergy Hockey Management to use my credit card to pay the agreed upon price of the program.

3. I have read and understand that Synergy Hockey Management will charge the credit card provided on the sign-up form the full amount plus applicable taxes unless otherwise stated by Synergy Hockey Management. Payment is non-refundable and non-transferable under any circumstance.
